Please Note: You must provide proof of 4th or 5th grade status. One page of a report card or progress report is sufficient. If the child is home schooled, the parent or guardian can include a note stating the child’s grade equivalent. Please bring this with you to the store, along with a completed Cold is Cool Application. Once you download the app, you will pay online and upload a photo of your child.
